Coordinated Care Initiative
California’s dual eligible integrated care demonstration is called Cal MediConnect and is one component of California’s Coordinated Care Initiative (CCI). The CCI is a new state program that changes the delivery of Medi-Cal, Long-Term Services and Supports (LTSS), and Medicare for dual eligibles and seniors and persons with disabilities living in seven California counties.
This website is designed to provide information and education materials regarding the Coordinated Care Initiative. Justice in Aging does not provide direct services to beneficiaries.
