Coordinated Care Initiative 

California’s dual eligible integrated care demonstration is called Cal MediConnect and is one component of California’s Coordinated Care Initiative (CCI).  The CCI is a new state program that changes the delivery of Medi-Cal, Long-Term Services and Supports (LTSS), and Medicare for dual eligibles and seniors and persons with disabilities living in seven California counties.

This website is designed to provide information and education materials regarding the Coordinated Care Initiative.  Justice in Aging does not provide direct services to beneficiaries.

If you are a beneficiary who received a notice about Cal MediConnect or changes to your health care benefits, please contact the Health Insurance Counseling & Advocacy Program (HICAP) at 1-800-434-0222 or Health Care Options at 1-844-580-7272.  If you have been enrolled in a Cal MediConnect plan and need assistance, contact the Cal MediConnect Ombudsman at 1-855-501-3077.
